· Advise us of injury or death to any of the insured people immediately.
· Complete a Claim Form and forward this to us.
· Submit Medical Bills to us and advise the date of return to duty.
· Advise us of the Salary of the employee concerned - provide last payment slip.
· Obtain a Doctor’s Report and submit this to us.
· In case of Death, obtain a Death Certificate and forward this to us quickly.
